Community Health Access Network (CHAN) assists Jackson county residents who meet the following eligibility requirements:

(1) have low household income (175% of federal poverty level),
(2) are between the ages of 18 and 64,
(3) are medically uninsured,
(4) cannot afford medical care,
(5) and have lived in Jackson County for at least one year.

The primary goal of the CHAN program is to continually develop and manage a coordinated system of donated medical services from medical providers for individuals that cannot afford medical care or access to physician services.

CHAN is NOT an insurance policy.  CHAN is a program to manage donated medical care from medical providers who have agreed to donate the services to patients before the services are performed.  Not all services are available through the CHAN program.  A limited number of providers participate in donating their services.

Community Health Access Network (CHAN) is dedicated to improving the health status of the residents of Jackson County through programs that partner with other agencies, organizations, and individuals from the community to improve health care for area residents.

Community Health Access Network (CHAN) is an organization that is coordinated, staffed and funded entirely through grants and donations from community members.

HOW CHAN WORKS

  • Each person must complete an application process to determine eligibility
  • A person can be referred to CHAN staff by an area physicians, hospital, or other  Jackson County agencies or organizations
  • A person can call the CHAN Income Eligibility Specialist for an appointment and instructions for the application process

The following are REQUIRED to determine eligibility:

1.  Bring a copy of the prior year tax forms, a current check stub, or other means of income verification (for each individuals living in the household).

2. Bring a bill with applicants name and physical address

3. Bring proof of age (if needed)

4. Bring all medications you are using (prescription, over-the-counter, herbal or others medicines)

  • Eligible person will receive a CHAN ID card to present to the medical provider at all medical appointments
  • CHAN clients are required to register with the Medication Assistance Program to determine eligibility for obtaining  medications available through the program
  • CHAN office staff should be notified if the patient receives a referral from the participating medical provider to another provider.
    • The CHAN staff will schedule the initial medical appointments to establish if the care will be donated to the patient.
    • The patient can make their own follow-up appointment after the initial visit if the medical provider agrees to see them as a CHAN patient.

For referrals to non-participating medical providers the CHAN office staff should be contacted for instructions before the services are performed.

  • Payment for charges for any services that are not donated will be the responsibility of the patient.

Patients are encouraged to volunteer in their communities to give back to the community.

CHAN’s FREE LUNCH and LEARN

The CHAN program and the NC Cooperative Extension sponsor a  FREE LUNCH   ‘n   LEARN on the Second Tuesday of each month in room 234 of the Jackson County Community Services Building.  The Lunch ‘n Learn discussion topics will focus on nutrition, health and a healthy lifestyle, and events or projects happening in the county.  Pre-registration is required for the Lunch ‘n Learn food preparation.  Please call 828-587-8259 to register.
