Child Service Coordination

The Child Service Coordination program available through the Jackson County Department of Public Health provides home-based services to children from birth to give years of age who are at risk for or have developmental delays or disabilities. Quarterly home visits are made which provide the basis for education, assistance in locating specialized services or benefits, and developmental. Medicaid is billed for eligible children.
 

Maternity Care Coordination

T
he Maternity Care Coordination program provides case management services, education, and resource and referral information to any pregnant woman who is Medicaid eligible.  Services may begin at any time during the pregnancy, from the time a positive pregnancy result is rendered until delivery.  Assistance is provided as needed in applying for Medicaid, locating housing, and obtaining other services such as food stamps or transportation.  There is no charge to the client for this service.
 

Maternal Outreach Services

The Maternal Outreach Worker offers home-based outreach, advocacy and support to clients of the Maternity Care Coordinator.  This intensive program provides weekly or bi-monthly visits to the mother-to-be from the time of the enrollment until the child’s first birthday. As with other social work programs available through the Jackson County Department of Public Health, clients are not billed for services obtained through the Maternal Outreach Worker program.
 

Safe and Snug Car Seat Program

The Jackson County Department of Public Health provides INFANT, CONVERTIBLE, AND BOOSTER SEATS through the Safe “n” Snug Car Seat Program for infants and children weighing up to 80 pounds.  This program is designed to offer residents of Jackson County a safe, secure way to transport their children in accordance with the North Carolina car seat safety laws and guidelines. Some fees may apply for issuance of car seat.
