The Child Service Coordination program available through the JCDPH provides home-based services to children from birth to five years of age who are at risk for or have developmental delays or disabilities. Quarterly home visits are made which provide the basis for education, assistance in locating specialized services or benefits, and developmental. Medicaid is billed for eligible children.
